D0220: Intraoral, Periapical First Radiographic Image
Definition
D0220 is the CDT code for the first periapical x-ray taken at a visit, an image that shows a whole tooth from crown to root tip.
Why it matters for the front desk
Periapicals are taken to diagnose a specific tooth, often at an emergency visit or before a crown or root canal. D0220 is billed for the first image and D0230 for each additional one at the same visit. Billing every image as D0220 is a common error that leads to denials. Some plans also limit periapicals or fold them into a full mouth series if too many are taken on one day.
